Proof of COVID-19 vaccination in NSW

Once you have received your first dose or are fully vaccinated (two doses) with an approved COVID-19 vaccine, there are several options to receive proof of COVID-19 vaccination whether you have a Medicare card or not.

Online if you have a Medicare card

Immunisation history statement

Your immunisation history statement shows all vaccinations you have had that are recorded on the Australian Immunisation Register. This includes your COVID-19 vaccinations.

COVID-19 digital certificate

The COVID-19 digital certificate shows only your COVID-19 vaccinations.

Your COVID-19 digital certificate will only be available to download after you are fully vaccinated (two doses) with an approved COVID-19 vaccine.

You can add your COVID-19 digital certificate to your smartphone Apple Wallet or Google Pay. Instructions are available on the Services Australia website.

Service NSW

Service NSW and the Australian Government are currently working on a way to show vaccination certificates through the Service NSW app.

Once certificates are available in the Service NSW app, they will integrate with the COVID Safe Check-in system, making it easier for users to prove their vaccination status when checking in to venues.

If you can’t get proof of your vaccination online

  • Call the Australian Immunisation Register on 1800 653 809 (8am to 5pm, Monday to Friday) and ask for your immunisation history statement to be posted to you. It can take up to 14 days to arrive in the post.
    • If you need an interpreter, please call the Telephone Interpreter Service (TIS) on 131 450.

If you don’t have a Medicare card

  • You can call the Australian Immunisation Register on 1800 653 809 (8am to 5pm, Monday to Friday) and ask for your immunisation history statement to be posted to you. It can take up to 14 days to arrive in the post.
    • If you need an interpreter, please call the Telephone Interpreter Service (TIS) on 131 450.
  • Add your COVID-19 digital certificate to your smartphone Apple Wallet or Google Pay using the Individual Healthcare Identifiers service (IHI service) through myGov.
  • You can get your immunisation history statement from My Health Record.
